your workflow

from context.
to coaching.

  1. 01

    Ask about the goal.

Find out what the client wants to change and what progress would look like in their life. A useful goal gives you more direction than a list of exercises they like.

  2. 02

    Understand the practical limits.

Collect training history, available equipment and weekly availability. Ask what has made consistency difficult before, so the plan fits the week they actually have.

  3. 03

    Review before programming.

Use the intake as a starting point for a conversation. Follow up on unclear answers, complete your appropriate pre-participation screening, and make the coaching decisions before assigning training.

From answers to a working brief.

Deload’s intake workflow connects those answers with program drafting. The trainer reviews the context and the draft, rather than handing the client’s answers straight through to a final plan.

An intake is part of onboarding.

Goal-setting and logistics do not replace health screening, informed consent or a service agreement. Keep those steps explicit in your process, and use the appropriate screening guidance for your clients and scope of practice.

your questions,
answered.

What should a personal training intake form ask?
Start with goals, training experience, current activity, available equipment, schedule and communication preferences. Use a separate, appropriate process for pre-participation screening and any necessary medical clearance.
Does an intake form replace a PAR-Q+?
No. A coaching intake gathers context for your work together. Use the current official screening materials and relevant professional guidance for screening decisions.
Can I use the intake template before Deload launches?
Yes. The free template is available now. Read it, edit it in your browser and download a copy without an account.
